Operation Doctor: Homemade Body Game
Build a giant paper 'patient' and use tweezers (or kid-safe tongs) to lift felt organs out of labeled holes. Every pick-up teaches a body part name — no buzzer, no timer, all fun.
Materials and setup
- cardboard or foam board body outline (3 feet long)
- markers, scissors, felt scraps
- kid-safe tweezers or tongs (all ages); metal tongs (Early Elem+)
- Kinder: jingle bell, masking tape
- Early Elem: aluminum foil, 3V coin cell + holder, piezo buzzer, insulated wire, electrical tape
- Upper Elem: above + green LED, index cards for diagnostic deck, timer
Parent note
This is the 'Operation game' capstone — the original inspiration for the whole Body & Wellness strand. Pre-K plays a pure fine-motor version; Kinder adds a bell for light stakes; Early Elem is a real working circuit (a great first soldering-optional electronics project); Upper Elem runs it as a full game design and engineering project across ~3 sessions. Store the finished board flat; it can be replayed as a warm-up for any anatomy activity (bw-003 through bw-020).
How this changes by age
Pre-K (ages 3–4)
Build a giant paper 'patient' and use tweezers (or kid-safe tongs) to lift felt organs out of labeled holes. Every pick-up teaches a body part name — no buzzer, no timer, all fun.
Steps
- Lay a large sheet of cardboard or butcher paper (at least 3 feet long) on the floor. Together, trace the child's body outline in marker.
- Parent draws 6 large organ holes inside the body outline: brain (head), heart (chest left), lungs (chest, two ovals), tummy (belly), bones (one arm), and skin-scrape (one knee).
- Cut the holes out with scissors so felt pieces can sit flat inside them.
- Cut 6 matching felt shapes to fit the holes. Child colors each one: pink heart, gray brain, blue lungs, tan tummy, white bone, orange scraped knee.
- Place the felt pieces in their holes so the 'patient' is complete.
- Give the child kid-safe tweezers or large tongs. Parent calls out one body part at a time ('find the HEART!'). Child uses tongs to lift the matching felt piece out without using their hands.
- Every successful lift = high five + parent says one true thing about that organ ('the heart pumps blood so your legs can run').
- Play again and let the child call the parts. Swap roles.
- Finish by placing all organs back in the patient and 'discharging' them from the hospital with a hug.

Kindergarten (ages 5–6)
Upgrade the body board with 10 organs AND a gentle 'buzzer' made from a bell on a string — if the tongs touch the edge of a hole, the bell rings! Learn organ names, locations, and one fact each.
Steps
- Start with the Pre-K body outline, or draw a new, bigger one on cardboard.
- Add 10 organs total: brain, heart, lungs (x2), stomach, liver, kidneys (x2), bones (arm), and appendix (one in the lower right belly).
- Cut holes to match each organ; the appendix hole should be the smallest for a challenge.
- Build a 'no-touch buzzer': tape a small jingle bell to the edge of each hole using masking tape so the bell hangs just into the opening. If tongs bump the edge, the bell rings — that's a 'try again'.
- Color felt pieces for each organ. Label each one with its name in big letters.
- Play in rounds. Parent shows a 'Patient Chart' card ('Patient complains of tummy ache — remove the STOMACH'). Child reads the chart, finds the correct organ, and uses tongs to lift it out without ringing the bell.
- After each successful surgery, parent shares one kid-friendly organ fact ('the stomach is a stretchy bag that squishes food for about 3 hours').
- If the bell rings, the organ goes back in and the child tries again after a deep breath.
- Score 10 successful surgeries to earn the 'Junior Doctor' badge (color and cut your own from paper).
- Cool-down: put all the organs back where they belong and have the child tell you the name of each one as they place it.

Early elementary (ages 6–8)
Build a real Operation game with a working buzzer circuit! Use foil, a coin-cell battery, and a buzzer to make the board light up and beep when tongs touch the edge. Then play with anatomically correct organ positions and quiz cards.
Steps
- Gather: cardboard body board, aluminum foil, a 3V coin-cell battery and holder, a small piezo buzzer (or LED + buzzer combo from a science kit), thin insulated wire, electrical tape, metal kitchen tongs.
- Engineer the circuit: line each organ hole's edge with a ring of aluminum foil. Connect ALL foil rings together with a single wire (this is the 'body' terminal). Run a second wire to the metal tongs (this is the 'tool' terminal). Battery + buzzer complete the loop: when tongs touch foil, the circuit closes and the buzzer goes off.
- Test the circuit with tongs before adding any organs. Fix loose connections with extra tape.
- Study real anatomy with a diagram: heart slightly left of center, liver on the right side under the rib cage, stomach upper-left belly, kidneys in the back (but draw on front for play), small intestine coiled in the middle belly, large intestine framing the small one, lungs flanking the heart, brain in the head. Place organ holes to match real positions.
- Make 12 Patient Chart cards. Each card lists a symptom and the matching organ ('patient has heartburn — stomach', 'patient has a concussion — brain', 'patient has a broken rib — bones near lungs', 'patient has a urinary infection — kidney', 'patient has jaundice — liver').
- Play 2-player: one person is the doctor (does the surgery), the other is the nurse (reads the Patient Chart and times each round to 60 seconds with a phone timer). Swap after 6 cards.
- Score system: +10 points for a clean lift, –5 for a buzz, 0 if out of time. Track on a scorecard. First to 50 wins the round.
- Post-round debrief: for each organ pulled, the doctor says one real function of that organ. If unsure, look it up together and add it to a 'Surgeon's Notebook' for future games.
- Engineering extension: add a green LED in parallel with the buzzer that lights when a lift is successful (wire it to a separate 'goal pad' the organ touches). Two sensors = two feedback signals.
- Cool-down: draw a post-op poster showing each organ's correct position and one function.
Learning objectives

Upper elementary (ages 8–10)
Design, build, and playtest a full Operation-style medical game — with diagnostic decision trees, a timer, a scoring system, and a rulebook. Then run a family tournament and iterate the design based on feedback.
Steps
- Phase 1 — Design: sketch a 2-page game design document. Define (a) win condition, (b) player count, (c) core loop (draw card → diagnose → operate), (d) materials list, (e) estimated build cost. Aim for under $15 in materials.
- Phase 2 — Anatomy research: pick 15 organs/structures. For each, research (1) location, (2) primary function, (3) one common pathology (e.g., appendix — appendicitis; pancreas — diabetes; gallbladder — gallstones). Compile into a 'Medical Reference Sheet' that comes with the game.
- Phase 3 — Circuit build: design a two-sensor circuit. Sensor 1: foil ring around each hole connected to a buzzer (failure feedback). Sensor 2: a foil target pad at the bottom of each hole connected to a green LED (success feedback). Both sensors share the tongs as the probe. Document the schematic.
- Phase 4 — Card design: create 20 Diagnostic Cards. Each card has SYMPTOMS ONLY (no answer on front). The back has the correct organ plus a 1-sentence explanation ('patient has severe pain in lower-right belly, fever, nausea' → appendix → appendicitis inflammation, surgical emergency). Cards should require reasoning, not memorization.
- Phase 5 — Scoring system: design a scoring rubric that rewards BOTH diagnostic accuracy AND steady-hand skill. Suggested: diagnosis correct = 10 pts, clean lift = 10 pts, buzzer = –5 pts, over-time = –5 pts, successful teaching of a fact to another player = +5 pts.
- Phase 6 — Write the rulebook. Include setup, turn order, scoring, winning condition, and 2 optional variants (e.g., 'Teaching Hospital' co-op mode, 'ER Rush' speed mode).
- Phase 7 — Playtest: run at least 3 full games with family or friends. After each, collect feedback using a simple survey (1-5 scale on fun, difficulty, clarity; plus 1 open suggestion).
- Phase 10 — Reflection: write a 1-page post-mortem. What worked? What would a 2.0 version change? What did you learn about the body AND about game design?
- Extension: map each organ to a real medical specialty (heart → cardiology, kidneys → nephrology, brain → neurology, skin → dermatology, bones → orthopedics). Add the specialty to the Medical Reference Sheet.
Learning objectives
- Design a physical game with a documented design doc, rulebook, and scoring system

- Research and accurately present 15 organs with location, function, and one pathology each

Safety and evidence note
Read the full activity before beginning. An adult should supervise tools, heat, food, outdoor work, movement, and experiments as appropriate. Completion records that the activity was done; the child’s explanation, work sample, photo, or demonstration is stronger evidence of learning than a completion check alone.
